Your medical history
Please advise the instructor of your class if you have any concerns about the following medical questions. We strongly recommend that you obtain your doctor's approval before participating in exercise. Not doing so increases the risks associated with exercise.
Is there any medical reason why you would need to modify, or exercise caution with your exercise program?
Have you ever had an injury or condition that may be aggravated by exercise?
Has anyone in your family ever had heart problems under the age of 60?
Are you currently or have you recently been pregnant?
Are you taking prescribed medication? If so, what?
Do you have or have you previously had, any of the following conditions?
□ High or low blood pressure
□ Epilepsy
□ Arthritis
□ Asthma
□ Bone or joint problems
□ Muscle tension
□ Sports injury
□ Raised cholesterol
□ Chest pain
□ Heart condition
□ Diabetes
□ Liver/kidney condition
□ Back problems
□ Any condition that may limit your activity
If you answered 'yes' to any of the above it is important that you discuss with your doctor the risks associated with exercise and the particular risks associated with your condition. Make sure you inform your instructor of these conditions.
Risk warning under s5M of the Civil Liability Act (NSW): Exercising at Mad Hatters Aerial Acro & Pole Dance Studio is a dangerous recreational activity and involves a significant risk of physical harm. In signing into a class you acknowledge that you are, by this notice, warned that you may suffer loss or damage to property, injury or death as a result of your participation in any exercise program, class or activity at Mad Hatters Aerial Acro & Pole Dance Studio. Mad Hatters Aerial Acro & Pole Dance Studio staff are not medically trained and are not in a position to provide medical advice. You must not rely on representation by Mad Hatters Aerial Acro & Pole Dance Studio staff as to your medical condition or as to your suitability to any aspect of an activity. If you are in doubt as to your capacity to engage in exercise you must consult a medical professional before engaging in any activity at Mad Hatters Aerial Acro Pole.
Release and indemnity: I hereby release Mad Hatters Aerial Acro & Pole Dance Studio, its servants, contractors, agents or employees from any claims that I may have against it in respect of any damage, injury, loss or death that may be occasioned by virtue of my participation in any activity at Mad Hatters Aerial Acro & Pole Dance Studio, howsoever caused.
Independent Contractors: Mad Hatters Aerial Acro & Pole Dance Studio uses the services of independent contractors. Mad Hatters Aerial Acro & Pole Dance Studio takes no responsibility for any act or omission by a contractor and you hereby release Mad Hatters Aerial Acro & Pole Dance Studio of any liability arising from a contractor's act or failure to act.

Terms and Conditions
1. Classes are taught in Ten (10) week blocks
2. If you are enrolling in a 10-week term, full payment must be made prior to the commencement of your first class. We will allow part payment of ½ upfront and ½ by week 5, with a an approved half term payment agreement and must be signed by both parties. This must be requested and approved PRIOR to booking and choosing the half payment option inline as stated in the note prior to selection online. This agreement is approved under special circumstances only. Casual payments must be paid online at least 2 hours before class starts and are only available for casual booking classes only. Spaces are subject to full time student bookings and availability of poles.
3. If you are unable to attend a class:
a) You will not be entitled to a refund
b) You may attend an alternative class, only if available and these are not garenteed and providing that class is not full; however,
c) You may not attend a class outside of the 10-week block*
d) You can organise to make up a lesson prior to the week/day you will be absent, as long as it is inside the same term*
4. You must not, under any circumstances, use material from Mad Hatters Aerial Acro & Pole Dance Studio for your personal gain without first receiving express written authorisation from Mad Hatters Aerial Acro & Pole Dance Studio.
5. If you have a new injury or are exercising under an Instructor who has not been made aware of your previous injuries, you must bring that injury to the Instructor's attention prior to engaging in any activity Mad Hatters Aerial Acro & Pole Dance Studio.
6. If you are under 18, your parent or guardian must read and agree to these terms and conditions on your behalf for at Mad Hatters Aerial Acro & Pole Dance Studio. I give permission for Mad Hatters Aerial Acro & Pole Dance Studio to use photos of me for advertising and marketing purposes. If you do not agree to this please inform your instructor at the beginning of you first class and whenever a picture is taken on you in class or on a show night, workshop, or event.
*Considerations will be made for students to attend make-up classes outside a term where advice is given in writing to Management prior to the class you missed for certain reasons outlined below. For Medical Reasons where more than one class is missed and you wish to make up a class in another term, a Medical certificate from your doctor is needed to arrange the make-up class. Considerations will also be made to those who are suffering Family death, illness or trauma where it has been discussed with Management.
